Supported Living
The
goal of supported living is realized for many of the people
we serve with the help of a Lifeworks Supported Living Program.
Because every person in our program has individual,
specific needs, Lifeworks offers a variety of supported
living options.
SUPPORTED LIVING PROGRAM
This program offers assistance and support
to individuals who live independently in the community,
either in a family home or in their own apartment.
A Lifeworks Case Manager provides services
that allow an individual to continue living independently.
Typically, these services are provided from
2 to 20 hours each week, and may include assistance with:

Advocacy:
Self-advocacy at work, home and in the community. Assistance
with entitlements and programs such as Social Security,
Disability, Medicare, MassHealth, The Ride, Section 8
and local housing.

Financial skills and
budgeting: Writing bank deposit/withdrawal slips,
rides to and from the bank, advice on budgeting.

Life
improvement goal planning: Setting and achieving
individual goals such as learning new skills (reading,
cooking, shopping, laundry, computer), community mobility,
joining clubs, voting, personal hygiene.

Medical Needs:
Arranging doctor’s appointments, getting to and
from medical appointments, help with follow-up care, supervision
of prescriptions, assistance with and administration of
medications, general wellness (diet, exercise, hygiene).

Shopping:
Meal planning, grocery and personal shopping, transportation
to and from stores.

Social Activities:
Finding clubs and organizations to join, planning and
taking part in social and planned group activities such
as bowling, miniature golf, movies, dances, sporting events,
travel, dining out, etc.

Travel: Researching,
booking and confirming travel arrangements with various
special needs travel clubs.
EXTENDED FAMILY PARTNERSHIP/ FAMILY
SUPPORT PROGRAM
This program supports individuals living at
home, ensuring that all identified needs are met and that
respite and support is available for family members. A Case
Manager works closely with the family, providing many of
the Supported Living Program services indicated above.
PUBLIC PARTNERSHIP
PROGRAM
The Public Partnership Program allows individuals
and their families to decide how state support funds will
best be spent in order to reach individual life goals. Self-determination
is the cornerstone of this program. Services may include:
tutoring, supported living (see above), transportation to
and from work, social events or personal appointments, job
coaching, tuition for specialized schooling, computer training,
driving lessons, assistance with life-enhancing purchases,
etc.
